Course Overview
The course “Incident Investigation & Root Cause Analysis” offered by the American Institute of Safety Professionals provides safety officers, supervisors, and managers with the tools necessary to investigate workplace injuries and ascertain the root causes per OSHA standards (29 CFR 1910, 1926). The importance of effective and thorough investigative techniques as a way of ensuring non recurrence and hazard mitigation is emphasized along with regulatory compliance. The course teaches participants structured methods of evidence collection and preservation, witness interviewing, contributing factor analysis, and root cause analysis of accidents, near misses, and unsafe conditions. The program lays out various evidence-based methods and tools including, but not limited to, fault tree analysis, causal factor charts, hazard identification frameworks, and various other data tracing techniques to root systemic organizational issues, helping participants trace incidents from surface level conditions. The course also emphasizes compliance within the scope of §1926.20 (General Safety and Health Provisions) which requires employers to initiate proactive investigations when incidents occur. Participants learn the best practices of documenting results, planning corrective and preventive measures, and implementing follow-up to check the effectiveness of the planned actions. These measures help organizations reduce exposure to risks, improve safety and implement risk accountability at every operational level. Taking this course enables safety professionals to guide incident investigations while remaining impartial, integrate lessons learned into safety programs, and develop a culture of ongoing improvement. In the end, the course sharpens the focus of an organization on responding to and adopting OSHA standards while maintaining employee safety and ensuring a compliant, safe, and productive work environment.
